If you are reporting a westie in need, please complete as much of this form as possible or contact the rescue representative closest to your location. Whatever you do – please DO NOT take your Westie to a shelter. Your Westie deserves so much more. Shelter environments are very stressful on animals. They can be exposed to kennel cough, parvovirus and various other viruses and diseases.
Submitting the surrender information form does not constitute an agreement of any sort with the Canadian West Highland White Terrier Club Rescue. It is merely the initial step in the surrender process. A representative from Rescue will contact you shortly. All dogs must be free of contagious diseases and all possible records and paperwork must be submitted for review.
Reports of any biting will be taken into consideration.
All information must be reliable and any references, if possible, should be supplied.
All rescues will be spayed or neutered as soon as deemed possible by a certified veterinarian, if not already done.
Along with the Surrender Information Form, please complete the “Release of Medical Information and Veterinary Records” and forward it to your veterinarian.
If your Westie is considered for our Rescue, we will ask you to sign a Surrender Contract. We would also appreciate a Surrender Donation to help defray the costs of re-homing your Westie.
Please note the Surrender Donation is not for profit. No one involved with CWHWTC Rescue is paid. We are all volunteers who do this in our spare time. Vet fees are expensive and we depend on donations to help with our costs.
